like when I, as a player, decided it was better to pick up the old lady with the crossbow and jump out of the 3rd story window instead of just taking the crossbow from her.

Depends on the session, what has previously happened, what I think the players are about to do. On average though, I try to give it a good couple of hours to make sure most of the army of ducks are in formation and appropriate number of rows.
