Grima Wormtongue, in addition to having the best name in a series filled with great names, is making sure Théoden embodies the major theme of the book. Wormtogue doesn't want Théoden to choose evil, he wants him not to make any choice at all.

"They'll be fine without hormones!" No, they won't. Many trans people can no longer produce their own sex hormones and will develop significant and costly health problems, up to and including premature osteoporosis. The rest will be tortured by the resurgence of the wrong hormones in their blood.

Alcott didn’t use male pronouns during life, but reading his words I see a mirror of myself, as someone who simply never conceived of such a thing as an option until I’d met other trans men, myself. As such I choose to honor him by referring to him as the man he consistently said he longed to be.
