Much research in human disease requires defining context - and this is very much a judgement call. Whose judgment is best becomes key question - do we want expert knowledge or LLM compilations or both? No easy and clear answers and if case by case will AI do well or just extend current ignorance?

There is waste at NIH and on NIH grants but much of the really wasteful money is spent on developing systems mandated by Congress to be more "accountable" with taxpayer $$. If we spent more on science and less on bureaucracy we would be far ahead.

You can collect the right data and do your analyses correctly but if you make wrong assumptions get a very incorrect answer.

There is overabundance of business thinking in all realms now. This is one approach, not the only one. It doesn't work very well for most non-profit enterprises. The business mindset uses a unitary metric for success. Need to include more diverse, long-term ways of measuring success

There is a relationship between how much you read and your ability to write. PhD students who don't read the literature are more likely to not only cite inadequately but they are more likely to produce mediocre papers that have limited utility
