By your responses though, you seem to be describing a 180° reversal. Why is a 90° swerve such a big deal? Why does that ruin it for you?

It's not real life. Everything on TV has some level of forcing one to suspend belief. It's the shows and moments where it pulls at your heart so hard, rather than your brain, that I guess I value more as a viewer. Yeah, if it's absolutely nonsensical, then I hear you, but this plot point made sense.

"Bad cliffhanger" is purely subjective. The whole premise is that innies are separate from their outies. So it stands to reason eventually, given the right circumstances (with the camcorder), they'd turn on one another. That final scene was heart-wrenching. Hate it for Gemma, but it was pure cinema.
