The fault is mainly at the top. The top DEMANDS result with shorter dev time, new unseasoned devs, multiple changes to genre of the game, chasing trends and GREED, releasing half broken messes, fix and add content as they go and high salaries and bonuses for the top. Not to mention layoffs.
