The wards those months were awful on a level that most people have never experienced and godwilling never will. The zeitgeist looked back on those years and cut basic science funding. We are lost

Every major healthcare system is integrating this technology into their practices - if you’ve been to a doctor in the past year it’s very likely there was an AI component to your visit. Overwhelmed hospitals are looking for any efficiency edge they can get, and it’s unambiguously good sorry man

I wouldn’t call it revolutionary, but AI medical scribe software saves a lot of docs I work with hours a day being stenographers for their patients, it lets them actually make eye contact and listen.
I’d never let a LLM come up with my treatment plan, but I’m happy to have it do the little stuff

I used to think I was sympathetic until I started to take care of trans patients in the hospital. There is so much to learn that was just totally absent in medical school, like the population didn’t even exist. Was a disturbing omission on reflection

Also now seeing aspirin is on his med list - no longer recommended for primary prevention of CV disease, now only used for ppl who have already had an MI or stroke. Did he have a stroke or a heart attack or something?

Also this is neither here nor there but reporting imaging results in the same list as the physical exam is very bizarre and not at all the norm in medical charts

Also rosuvastatin is a high-intensity statin - paired with zetia which is an adjunctive cholesterol lowering medication, this reads like he has a serious cardiovascular risk that a doctor is aggressively managing. Would not surprise me if they are concealing a larger health issue

Also a carotid ultrasound is not reported as “normal” or “abnormal” ever in any documentation I’ve seen or written. It’s always a measure of the percentage % of stenosis and calcification in the arteries; there is no person over 70 that doesn’t have at least a 5% restriction

On rounds before we start giving someone steroids we absolutely positively want to make sure they do not have an infection, because steroids suppress your immune system and you can make someone septic by accident

For sure I know a lot of people who do a disclaimer like “I’m sorry I have to be typing while we talk” - this is actually one of the few probs AI actually solves- lots of people are paying for ai scribe software that listens and writes for you nowadays

I try not to write in the room as much as I can, but it helps me give better care if I write down what patients tell me
