I was involved in a large clinical trial for a new drug that was supposedly going to revolutionize the treatment of anemia in patients with kidney disease. I was on the academic side so I had no skin in the game (financially speaking). The trial involved 7000 patients worldwide and cost $1-2 billion

This was only one of maybe 5-10 different trials by the same company of the same drug and 4 other companies were also developing similar drugs and performing similar trials. They all thought they were going to be successful and make them a lot of money.

One successful drug funds the development of 5 to 10 other drugs which might never pass initial testing, much less trials.
