If the image is the point of the post, it's clearly not just decorative and needs alt text.
Always include an alt attribute (alt=””)

Here on Bluesky, if you add an image to a post, it's not decorative and you should include alt text.

This page mainly describes alt text and its use for web pages, which does not mean all that is told there also applies for alt text in social media like BlueSky.
The hint with the period is a good one, but usually something like "decorative" images does not exist here.
Web pages can have images for decoration, but here no image can be set as a background for example.
All the screenreader user gets is an info that there is an image
If you don't want to have your image carry any information or message, don't use it for decoration here, better omit the image than omitting the alt text.
And there's absolutely no such thing like functional images here, like buttons, for obvious reasons.
