Whatever puts the brain worm in there hard enough to start writing it. Often it's the characters that start the internal ball rolling though. If I want to play a character in the setting, then I'll build the setting lore and system around it.

Reverse role, eldritch horror. You play a dead god, greiving and broken, traveling to different realities where the fabric of said reality will warp history and the people within to reject your presence. Isolationist body horror, where you are the monster from beyond, trying to process your death.
