Hadn’t heard of “Northern OMI” either. Need to look at the paper for any references to it. Don’t know if this paper will move the “doesn’t meet criteria” issue-I hope it does. Will have to reach/convince the cardiology audience. Therein lies the rub.

I like that idea. They have to learn to make decisions but know they have backup. It’s a little different in the ED since we’re right there with them 24/7, so sometimes they need a little nudge to do it. Always great to see them spread their wings and develop (appropriate) confidence.

There’s also concordant STE in V4, so don’t even need to use the modified criteria. Should be slam dunk for emergent cath.

Looks like high lateral OMI-aVL very suspicious STE, lead I suspicious also. Lots of STD, the inferior STD reciprocal? Good learning experience for PG1 to see this.

Maybe pitch the alternative. Ours didn’t care as long as u “checked the box” and had your ACLS. They actually dropped the ACLS requirement if u were BC in a critical care specialty (EM, Peds Crit care, Pulm/CC, and I believe also Anesthesia).

Try ACLS.com. All on-line and easy to do. Our hospital has accepted this for years. Having to show up in person for these at this stage of your career is ridiculous.
