NEW: In November, I wrote a Guardian column headlined 'How to Survive the Broligarchy'. Pretty much everything predicted has now come true. What I'm launching today is part handbook, how to protect ourselves? And part manifesto, how to fight back? Please join me. 1/ open.substack.com/pub/broligar...

This is a very excellent thread you should read to the end.

Once upon a time, some villagers came across a large, ornate wooden box in the local woods. On the side of the box was a message: “Whatever money you put in this box will, in future, grow to become 2.5 times larger.” 1/

A short #IBD bluetorial about mucosal bridges as complications of J pouches. These can occur 2-48 months after a J pouch surgery. Patients generally complain about an increase in evacuation difficulty, like "something is blocking the stool" 1/4

No one can truly capture what cuts like the ones announced Friday might do to American medical research. But I decided to try, in terms that a general audience might understand. I hope it's useful. open.substack.com/pub/karagavi...

Serum IL-6 likely picking out the JAKi responders in ASUC #IBD Stool TNF early, and TNF staying up by d3 - time to move on from steroids/TNF #ECCO2025

Are we approaching a ONE biologic -> JAKi era? #ECCO2025 In the US for sick #IBD patients, that means biosim IFX (or IFX/Aza) then Upa (or tofa when generic in 2026). This is the route to get to JAKi fastest. If the patient is less sick biosim Uste/Vedo/Risa -> IFX -> Upa/Tofa may be the way

Well, that never worked. Even the Takeda folks knew it was a dry well. More is not always better. If Vedo is not working, move on in #IBD. #ECCO2025 @bealoquebea.bsky.social

I really liked this idea of using a histogram as a legend in a choropleth map (since land isn't unemployed; people are), so I made a little guide to doing it with #rstats, {ggplot2}, and {patchwork} www.andrewheiss.com/blog/2025/02...

After 2 long years, I finally wrote a new blog post about git stashes! I am CONSTANTLY switching between branches at work, so I learned more than I would have liked about stashes, but admittedly, I can't work without them now! 📖 Git Stash for Newbies on The Tidy Trekker:

Bacteria on the wrong side of the mucosal barrier in Crohn's disease. #IBD pathobionts more often found in the lamina propria. Barrier function in CD is not the same... academic.oup.com/ibdjournal/a...

Salmonella infection associated with subsequent #IBD diagnosis in a population != Europe. One of several infections (Shigella, Ecoli, Campylobacter) associated with #IBD. Could vaccination against these enteric infections in at-risk kids (FDRs) reduce IBD incidence? academic.oup.com/ibdjournal/a...

Yet another meta-analysis in UC, makes the case for: - get to Upa quickly in sick #IBD - not much diff btween IL23s (uste biosim soon) - no incentive 4 head-to-head trials - b/c likely 2 show no difference, unless folks who make upadacitinib are feeling spicy. www.cghjournal.org/article/S154...
