the disruption of HIV treatment for underserved countries and their big populations of PLWH ... I worry about development of drug resistant virus, even if the disruption is short enough to not cause immediate mortality concerns. And... what about other STI's?

Anthem's statement simply says that they expect correct usage of CMS's regulations about time units (ie startup and post-op times, medical complexity, etc). CMS has not to date applied time limits to anesthesia care.

you presented the Anthem proposal as hewing to the existing CMS regulations. Which it doesn't.
Anthem wants to limit anesthesia time units to the expected procedural duration. That's not how CMS sets out anesthesia payments, so it's a misleading - though attention-grabbing - statement.

Anesthesiologists don't bill per procedure; we've always been a time-based specialty. If you work in this field, as you say, then you would know that. CMS manages CPT codes for procedures and proceduralists, but has rules for the anesthesia time units.

I’m a critical care anesthesiologist because I like it. When somebody has pain because their abdomen was sliced open for cancer surgery, or their airway is about to swell shut from a house fire, I’m the guy. If it makes you feel better to think I’m doing it for the money, go ahead and diminish me.

We don’t get to pad our time. We get paid for the time we spend caring for patients. That data is carefully monitored by both the hospital and our billing services; it covers the time we spend in the OR for each procedure, with pre-Op and post-op time.

the move is also poisonous in that Anthem specifically is aiming to "split" the anesthesiologists and the proceduralists. The goal is to incite dissension and discord - otherwise why penalize the anesthesiologists for a prolonged surgery? Why not pay the surgeons less instead?

ever since one of my Neuro-Critical Care friends told me to walk the walk about Chronic Traumatic Encephalopathy, I have tried to care about both college football and NFL as little as possible - no matter how my favorite teams are doing.
