To me the genre is basically an update of the noir detective story. The little guy just trying to scrape by within the confines of an overwhelming and corrupt system. Without the 80's future esthetic it's just dystopian fiction of any other variety.

Insurance will only pay a portion of the charged amount. Add in the cost of having to submit multiple claims to even have a chance of getting reimbursed and you have an idea of why the prices get bumped up so much. Check on the prices of paying out of pocket directly, you'll see a vast difference.
