If you look closely where the sun is coming up you can see land. That's Canada 😊

Sorry I'm a tease.

There is currently a clinical trial looking into this and it should publish results in late 2025 or early 2026. If confirmed this would be yet another remarkable effect of glucagon agonists so stay tuned!

That both of us essentially match the 10% claim of this paper and would go a long way explaining why people lose so much weight on retatrutide. A 10% increase in RMR is *huge* especially when you consider that normally when you lose weight your RMR *DECREASES*

A few months ago while still in the retatrutide trial myself and another trial participant paid to get our resting metabolic rates(RMR) measured by indirect calorimetry. My actual RMR ended up being 9.5% higher than predicted. The other person was 11.9% higher. While this is an n=2 it's curious...

The authors note that this effect could potential increase energy expenditure/resting metabolic rate by about 10% and that glucagon would help drive this effect. Well a glucagon agonist is 1.) lipolytic and 2.) mimics effects of fasting
comment in response to post
The authors hypothesize that the cycling of lipids from free fatty acids(FFA) back into di and tri- glycerides and back to FFA 1.) is energy intensive and 2.) designed to protect the body from the toxic nature of excess FFA while using lipids for energy in times of fasting/starvation

Worse is that for many of these patients they could prescribe tirzepatide, get their weight *DOWN* THEN do the surgery once they're less of a surgical risk. It's ridiculous and plays into the stereotype about weight loss & is glorified fat shaming IMHO

The worst part is that I'm so close to Canada that half the time my phone gives me the forecast for the nearest Canadian town. So close yet so far 😕😐

I said months ago I'd have to take my currently 3 month old infant to Canada to get fully vaccinated....and I hate that it looks like I'm going to be right. As a nurse practitioner this is quite literally genocidal behavior.

That section of the W-P trail is essentially never dry! I'd usually take the detour around it when I ran out there

Definitely on the SGLT2 needing more education. At my own clinic one of the doctors didn't realize that until I told her and then showed her the indications

That's HGH, and probably a shit ton of other gear/anabolics. Bet he's got some hardcore left ventricular hypertrophy (aka enlarged heart) The tiny legs just complete the picture of an insecure chud
comment in response to post
I've seen chickens with thicker legs than that.

I pride myself at daycare dropoff in alternating between looking like a ragged unshowered excuse for a human one day and then next day show in my clinic business casual looking sharp. Keeps everyone on their toes

This was my question yesterday as well. Noticed ~25% of patients were on GLP-1RA I'd love a subgroup analysis of albuminuria reduction + GFR slope for that group.

It's also a tiny increase in actual risk. From 0.1% to 0.2% and it's in an already elderly diabetic population so the risk of a confounder is high. I'd like to see this done as a prospective study.

Thanks! I only reverse sear for really thick cuts or if I'm doing a prime rib roast I use the @aragusea.bsky.social flip every 30 seconds method for a thinner steak such as a flat iron. Especially because my induction stovetop cooks so hot. It'll burn otherwise.

Morty. I lost 75 lb and even though I run, and lift I have no ass anymore and no amount of squats is bringing it back... Much to the disappointment of my wife

It's funny you interviewed DiMarchi for this. He warned in 2012(!!) not to approach this space with something like CagriSema. Too complicated/risky to create separate molecules. I think as we can see he was correct. Otherwise great article!

Yeah I don't blame you I did the same so I wasn't rebuying my wardrobe twice. Same thing for my wedding ring. Went from size 9 to size 8 finger but I waited to do that until I had stopped losing weight.

Congrats! Keep it up! Have you had to change pant or shirt size yet? 30 pounds seems to be about the threshold for that.
comment in response to post
I need to see this trial last longer specifically for the GFR in the combo group. I'm curious why it didn't rebound more. If you have any thoughts, I'd love to hear them

Jane I'm gonna need you to review the Ruby Red Grapefruit Monster because that has become my go-to even over Celsius. It's just *good*

Omg I feel this. I forgot to add cat food to the list of things to pick up at the grocery store, I remembered this morning because there was no food left to feed them 😂😂

My wife will point out I'm one of the smartest humans she knows but I'm horrible at remembering to do simple household tasks without literally making a checklist on my phone or on paper 🫣
