This is an excellent thread that reflects a LOT of what we are dealing with academically and clinically and socially in our service. Damned if you do, damned if you don't, and damned if you think it or say it - or don't. Not an easy space to work in.

Long COVID is a heterogenous disease that has not been well described yet, and research is still required to be able to quickly phenotype patients and guide them towards appropriate management (drug and non-drug) strategies

I found that discrepancy odd too. The improvement from baseline in the intervention group was larger than the specified 10 MCID but it was the difference between groups that was 9.2. The intervention is also interesting (supplement 3) as lots of the CBT could be considered good clinical comms.

During wave 1 of #covid in the UK we used a very similar (?!) van to start the country's first Long Covid clinic (didn't know if was going to be long covid when planning) Van usually used by Find and Treat team screening for TB out of @uclh.bsky.social

The REGAIN study cannot be applied across the full long covid cohort as it was only those with moderate or severe index illness included. There are significant differences in presentation between post hospital and community managed cohorts. Better defn of Long Covid to allow accurate study needed ++

One of my take homes would be that the initial investment in COVID research ignored the concept of Long COVID, and that was a huge missed opportunity. We've been running to catch up since. It is a crying shame.

I still work with Tino - he was my SHO when I was a house officer and because he went off to do his PhD I ended up as his Consultant and now we are Consultant colleagues... Funny old world.
