There's a lot of that in cataloging, due to inertia but also lack of staffing to make revisions to non-public facing codes or captions. In LC's Class Web, there's still many old (no longer valid) classification numbers that still have old terminology as the main display caption. Here's an example.

Many such cases (for example, "gb" for Kiribati, "uv" for Burkina Faso). They don't think it's a priority to update codes the public doesn't see.

Also a good time for libraries to rethink paying vast sums for unreliable library software from companies like Ex Libris Group owned by Clarivate. There are other ways to run our libraries.
