John Fetterman was meeting with reps from a teachers union when things quickly devolved. He began repeating himself, shouting & questioning why “everybody is mad at me,” “why does everyone hate me, what did I ever do” and slamming his hands on a desk. It is raising concerns about his mental health.

But, his cognitive decline may be preventing him from realizing it.
Senator Fetterman needs to resign and get help, respectfully.

He spent six months at Walter Reid right after he was elected for severe depression. From someone with the same diagnosis, as well as bipolar 2 I can tell you that we probably don’t know the whole story about what he is dealing with.

He has a mental illness. People who act like this with a mental illness need help. You don't have to help but take it seriously. People who act like this without a mental illness are the ones you should make fun of.

I do want to say, though, that personality changes do arise from severe brain trauma. This includes entire worldviews. People have gained and lost religious beliefs, for example. So this may well be due to Fetterman's stroke.

I get it. My position typically eats the “I need help” folks, because they don’t ask until they’ve passed the point where they can swim out of the current.
Showed him to be inappropriate and lacking judgement in regulating his behavior... if only for the one day. He's offside.
You need to let everyone see what is going on for John to get the help he needs. It probably feels like you're protecting him by hiding his problems, but he needs medical intervention before he hurts himself or someone else.
Just an observation.
If they are struggling with diminished capacity that's probably scary and they want to keep to things they already know and understand.
