A game where there are like a hundred small waterfalls, each having a key behind them, and one huge waterfall with a giant chest that needs 100 keys to open. Once you open the chest there is only a note inside that says "Ha ha, wasted your time".
Make it so if they go back it happens again, but the third time it gives them a Steam achievement, a completely useless inventory trinket and ALSO does it again
I think that Noki Bay in Super Mario Sunshine had one of those that knocked you off the giant cliff face you just spent 10-20 minutes climbing and exploring
You have to dodge random lightning strikes 200 times in a row in Final Fantasy X to get a very special item; people will very quickly become obsessed with trying to dodge the punch so they can get the imagined treasure

As a gamer I would find this super hilarious. The perfect balance between "oh something behind a waterfall so original" and "what!? Nani the heck where's my stuff behind the waterfall?"
Extra evil, have the cave be around halfway up the falls, with a kind of annoying path leading up to it. The sucker punch sends you all the way back down.
NPC near waterfall you cannot kill that allows you to see a cutscene of what happens when you enter said waterfall.

who couldn't resist the urge to find an Easter egg behind a water fall, then as soon as they get their, their fucking punched so hard their socks fall off. great idea
Now, for visual clarity, does the boxing glove extend out beyond the waterfall when it hits or do you just see your character get launched back out of the waterfall?
*proceeds to spend hrs to either try and stop the boxing glove or to try and avoid getting hit when going behind the waterfall bcuz there might be treasure behind that boxing glove boss
If you hit six frame-perfect inputs in a row, on one waterfall (randomised per save file which it is), you evade the punch and get the most OP item in the entire game
