Not only can different stimulants and sedatives affect the body in vastly different ways, a lot of drugs don't fall into either of those categories at all.
Ketamine is not strictly an upper or a downer. In low doses it has stimulant effects and in high doses it’s a sedative. Regardless, it causes strong disassociation, and this to me looks like a textbook case of someone fighting for their life to maintain composure while completely disassociating.
This is a fair point, most people who have used Ketamine recreationally have taken huge doses. I'm sure Musk has at some point. But yeah, he may have found his 'sweet spot' on a lower than even therapeutic dose, where he dissasociates enough to 'be detached', buzzed, but still functional.
