These people got the visas to do care work and aren't doing it, so where are they?

Whilst I definitely agree that wages need to double (at least), please respect that making sure people do not die is already a skilled job.
Having hard-working highly-skilled people in these roles is precisely the reason this country relies on immigration.
The idea that we have a high quality British workforce waiting in the wings would be laughable if it weren't so xenophobic.

Brits don’t want to work in care homes. Plus when there is a scandal of cruelty/neglect it is often British care workers involved.
The emphasis here is not on changing legislation to protect care workers from being exploited by companies but by blaming foreigners and preventing them from working.
