One of the biggest problems with this is that very few insurance companies will carve out the pharmacy benefit. It makes them too much $$$.

If you want to independently contract pharmacy benefits you end up with limited insurance options. Insurance companies are are as bad as PBMs

Mark, you are so right about so much of this, but miss the real issue. Employers doing a better job isn’t the answer. It’s time for employers to get out of the health insurance business. It’s an accident of history that should be corrected. ICHRA plus a robust public option is the way forward.

Seem far fetched? Consider what happened with DB pensions. When employers decide: 1. it’s not in their interest to keep providing a benefit, 2. there are no structural obstacles, and 3. there is a viable alternative, they will act. One big ER adopting ICHRA and the dominoes will fall.

How would health care providers get paid? They'd have to bill the single payer and someone would have to process that claim and pay the provider. So, I don't agree that the millions would be unemployed. There might be some job loss, but not all currently employed. We'd still need prescriptions, too.

As a mental health provider I have no power. All the contracts are weighted to their advantage. I have no bargaining rights, no insurance incentives and must take what they offer. Some contracts pay me less than a living wage after I consider my 30% tax rate. I will always be poor.

Mark isn't wrong but is exhibiting a degree of tunnel vision. Direct contracting requires volume and scale enough to warrant engagement. LOTS of employers can't generate critical mass to this end.
