I think that covers it.
There's a character in Catch-22 who is supposed to be taking his turn to go on a bombing mission (but isn't), when the plane he's supposed to be in goes down. He's officially reported dead and has no way to prove otherwise, because the paperwork channel doesn't exist. He's stuck in Italy.
