Our party murdered a wizard in his tower because we didn't even ask him any questions. The locals told us he was the source of a plague so we attacked him in his tower. Turned out, he was actually trying to brew the cure. The disease source was never discovered. Oops.

The party was stopping a cult from sacrificing innocents to summon demons (Typical Tuesday). They couldn't defeat the head cultist or escape fast enough, so my player took one of the hostages and killed them himself. That hostage...was the best friend of another PC...and he knew that when he did it

It led to a FANTASTIC in-character argument (that was reiterated to be all good above table) where the party truly debated whether killing one innocent for the greater good was worth it morally. A literal trolley problem, if you will

Well. So there is this god in the game we play who is corrupting everyone and everything, not only through his acolytes, but also through cursed stuff (artifacts, everyday things, even places and buildings could be cursed)

One of our players got cursed. That was really bad. He... he "had to go" QwQ

We knowingly left a player character behind to cover our escape and die. There was just no other real option.

It wasn't a case of any one of us staying behind, either, else I'd have stayed myself.

No one else was as large or fearsome a combatant, nor could hold off the pursuit as long as him.

So I set fire to the rear of the building, protecting his flank, and promised I'd get the others to safety as I locked and barred the bolt hole.

He killed almost a dozen before he bled out, buying us nearly an hour to escape through the city sewers.

The GM gave him a brilliant death scene.
