But as a pilot, you know that there is rarely a single cause to any accident (or fortunately, a close call). Pilot error, sure - but any decent investigation would look for possible contributing factors. Could signs & runway maps etc. have been more clear, for example?
Also realize there are daily and monthly errors of this sort. Just because we are reporting on them more does NOT mean they did not occur previously. Now actual crashes? That’s an entirely different matter …..
