And that's how it is seen from a business owner standpoint.
But the real question is why PBMS exist at all. They are just a specialized form of health insurance creating a 7B profit incentive/ profit tax the rest of us have to pay.

CVS Health is the largest PBM (21.3% market share), followed by OptumRx (20.8%), Express Scripts (17.1%), and Prime Therapeutics (10.3%).
why is YOUR state doing business with them? Those are tax dollars & that should go to the lowest bidder

And if you do not have insurance, you pay what Medicare pays for the drug plus a small fee ($5 to $10) to Medicare.
Makes me crazy.
I would bet a lot of money that before the year ends, the right to negotiate drug prices by Medicare that is supposed to happen in 2026 will be torpedoed.
Needless to say I paid out of pocket.
It had a lot of pages.
This and the limits on investment in Chinese AI is how the last budget bill lost pages.
