1. The section of the official Wheel of Time game, where you have to travel through The Ways. If you know, you know. That shit was ATMOSPHERIC.
2. The original Dying Light, in its janky, experimental VR mode. Most exquisite emergent jumpscares of my entire life.
Doesn't help that I technically wasn't allowed to play it, so I was playing it at 1am after my parents went to sleep in a creaky house orz
The scariest game i have actually completed is probably Dead Space 2
Had nightmares and Still came back

Ngl it was pretty awesome
Though, my answer is Phasmaphobia. Got talked into it, and playing with a group of friends takes an edge off the scary a bit.
But I like really atmospheric/melancholic games, too. The ones that stay with you after the credits roll. 😄
The problem was, I was waaaaay too young for a game like that 😂 yikes.

Anxiety inducing
first time playing on ps2 when you round that corner in the dark motel hallway and pyramid head his just staring at you from behind a locked gate glowing red has never left me
It's so good and so creepy
That game scarred me for when encountering other players unexpectedly.
Respawning enemies, weapon degradation, scarce resources, and a progression system complicated enough that you're never quite sure if the build you're speccing to will make the game unwinnable 5 hours from now.
(And the game WILL 100% allow this to happen.)
The sins of the past come to life and attack everyone in an isolated island prison.

Playing alone, dead of night. Cleared a cave, found loot, had been managing inventory for a few minutes.
Then *this* appears in the corner of my eye. 😱 First I'd ever seen one.
Scarier still, it had stalked up silently. No moans or groans.
The music is so amazing that when a DD track starts while I'm running Pathfinder, I feel cold sweat and my heart rate increases as my body goes in survival mode 😂
After a few tries to continue with a successful run (read: good enough) the music got added to my playlists but it soon turned into a blurry mess to my ears. To this day, I don't understand why I dislike it now.
Also I think there was some glitchy stuff that somehow helped it be weirder.
Id say it was more apprehension than fear.

I was 8 years old with a brand new computer setup in the basement. I finally finished installing the game (3.5" floppies) and drivers for the joystick (yep, no mouse) around 10PM.
About 90 minutes later, an imp scared me so bad I screamed and broke the joystick in half.
The only time I nearly had a panic attack while gaming
