In olden times, people experiencing severe and long-lasting diarrhea were (naturally) shunned, ostracized and driven from the village, to die hungry, cold, alone and dehydrated. This is why they died. Not even the wolves and insects would touch their smelly, diseased corpses!

I know that this isn’t serious per se but as someone who recently contracted severe food poisoning (in another country hence my stomach not familiar with the bacteria I encountered), it probably isn’t from the runs but from whatever is causing them obvi but I could’ve seen how you’d die without meds

You miss part of the equation. This was not a standard case of the trots, this was a massive case of dehydrating diarrhea on bodies beaten down by multiple rounds of various diseases. With the inability to replace those lost fluids via IV.
I have no problem with statues honoring fallen Confederate soldiers, as long as the statues are historically accurate and depict them dying while shitting their guts out, as most did.

No finding clean water to drink was an issue and medicine sucked! They didn’t even know why it happened. People died because they didn’t replenish their fluids. It also didn’t help that the “doctors” back then would just slice you open and bleed you or attach leaches for every medical issue!
I remember a poster in one of my classrooms growing up. It had all the presidents and what they died of. There were a few that said diarrhea, and of course, as 9/10 year olds, that was the most interesting thing about the presidents that we had learned.
10th grade American History, I was excited when we got to the section in our book on President Polk, my ancestor. We were reading in class - 11th president, blah blah, expanded US territory, blah blah, died of DIARRHEA! Covered my head in shame. It's the main thing I remember from American History.

Was the diarrhea watery or bloody? Was there pus or mucus in it? If it look like rice-water that means you’re losing 1 liter of water/hour ~50% your weight and will most likely die in 48 hrs

The most important intervention for diarrhoea is drinking lots of clean water, ideally with electrolytes, so it's not surprising places struggling with that would struggle with the disease
The work Gates has done with orgs like Partners in Health is incredible. They funded a national HC system in Rwanda.There was an area where 1 Dr. served 250k people and Gates/PIH built a 2 story hospital with clinics and ambulances. I stayed in tiny room and there was a pic of Gates in the same room
